HaysMed Orientation

Step 1

We hope you have a wonderful experience throughout your placement with our facility. This orientation process will introduce a general overview of our culture, values and various policies to assist you in becoming a productive member of our team. We strive for positive patient outcomes from patient safety, quality care and top notch customer service.

Please take time to review the following information to get you started. We recommend using Mozilla Firefox or Google Chrome web browser to complete this orientation process.

Step 2

Now that you have reviewed the material above, please complete each of the following forms by typing into the appropriate fillable area, print and sign.

You may return in a pdf format to debbie.mclaren@haysmed.com or fax to 785-623-5108 unless otherwise directed.

Please provide a copy of current TB skin test within the last year, copy of current CPR card, and current seasonal flu vaccine (October 1- March 31). You may be asked to provide and complete additional pre-boarding health assessment requirements in addition to this packet.

Please note: We do not accept electronic signatures at this time. We ask you to carefully review before sending that all signatures are present on each form. Failure to complete correctly could result in delay of processing.
