Internet Access
HaysMed patients and their visitors have access to high-speed internet via three Wi-Fi hotspots. The access points are located in the cafeteria, surgical waiting room and ICU waiting room. Massage
HaysMed offers on-site massage therapy for both patients and visitors. Massage therapy can help you relax, realign and rejuvenate. There are many positive aspects to receiving massage therapy on an ongoing basis, and with the busy lives we lead, we can all benefit from a little stress management. HaysMed Art Collection
The Hadley Foundation Art Collection at HaysMed includes over 408 works of art created by Kansas artists. This collection of original work began in 1974 at the Hadley Regional Medical Center and continued with the merger of St. Anthony Hospital in 1991, when the two hospitals became HaysMed.
To take a self-guided tour of the Art Collection located in public areas around the hospital, stop by a HaysMed Information Desk for a helpful brochure. Guided tours may be arranged with advance request by calling the Volunteer Services Office at 785-623-5890. While art pieces in restricted areas are not generally open to the public, special arrangements for viewing, without interfering with medical services or administrative operations, may be made in advance.
The Art Collection has been funded primarily by the Hadley Foundation. HaysMed Volunteers funded the Meditation Garden, and the Ross Foundation funded the mobile in the Miller Medical Pavilion lobby. Additional funds have been provided for the purchase of artwork from other resources. Several works of art have been given as memorials with the approval of the Visual Arts Committee. The Art Collection is on permanent loan to HaysMed.
HaysMed building décor in itself is a work of art deserving a tour. The Healthcare 2000 Interior Design Concept is a Northwest Kansas motif which is used in the design concept for the decor and furnishings at HaysMed. Native limestone boulders are combined in a two-story atrium entrance composition incorporating a waterfall and plantings. Colors and textures indigenous to the area are used throughout. The five horizontal bands of the wall covering repeat the limestone strata.
Art Collection Selection Process
The Visual Arts Committee follows established guidelines to select the art. Only well-established professional Kansas artists qualify for consideration. Included in the Art Collection are paintings, drawings, prints, sculptures and fabrics. Currently, 22 Governor’s Artists Award winners are represented.
The Hadley Foundation Art Collection received the Governor’s Award for Art Patron in 1994. The Hays Arts Council recommendation noted the overall commitment of the Hadley Foundation in the support and purchase of art. The Governor’s Award acknowledges the importance of the Art Collection within HaysMed and also that HaysMed has become a major repository of works by Kansas artists.
National health organizations encourage and support the integration of “the arts” into the healing process. Indeed, the Hadley Foundation Art Collection pioneered the concept of utilizing original art in a healthcare institution. The Art Collection recognizes the therapeutic value as well as the aesthetic enrichment of the visual arts for HaysMed. Because health involves both physical and mental wellbeing, creating an atmosphere which enhances that spirit is an essential aspect of HaysMed’s commitment to the arts.
The Arts and the Healing Process
The Hadley Foundation Art Collection is a pioneer in recognizing the aesthetic and therapeutic enrichment of the visual arts in a healthcare setting. That realization has been the historical basis and the focus of the Healthcare 2000 Interior Design Concept. National health organizations encourage and support the integration of “the Arts” into the healing process, and it has existed for years at HaysMed.
Therapeutic Value of Art
The aesthetic value of the Art Collection was the initial goal. However, that goal was achieved and surpassed by the realization of the additional therapeutic value it offered to patients, families, visitors and associates. Stroke patients who have particular difficulty with initiation of spontaneous speech, or with unilateral neglect of one side or the other, respond well when introduced to the variety of displayed artwork. It has proved to “soothe the soul” and provide comfort during difficult situations.
Family members can relieve stress by reflecting on the artwork displayed in the halls. Visitors remark upon the impressive variety, quality and added dimension that the Art Collection contributes to the HaysMed experience.
HaysMed associates refer to the Art Collection with pride and often develop “ownership” of favorite pieces. Administration receives favorable comments on a regular basis and includes the art tour for visiting dignitaries.
Expansion of HaysMed Art Collection
The DeBakey Heart Institute and Rehabilitation Center in the Bickle Pavilion was completed in the spring of 2005. Expanding the Art Collection into the new area was funded by a Hadley Foundation grant. One of the features the grant allowed was placement of original art in each patient room in the new building.
Noted Kansas artists are represented in the Dreiling/Schmidt Cancer Institute. Consideration is given to the subject matter of the artwork in this area of medical care. Included in the collection is a major work of art known as the “Leaves of Life” recognizing the journey of those whose lives have been touched by cancer.
The Hadley Foundation
The Hadley Foundation was established in 1991 to provide support for the activities and mission of HaysMed. Its resources originate from the estate of the late Mark Hadley whose ancestors were early settlers in Ellis and Rooks Counties and successful in the ranching and oil business. The Foundation is governed by a board of eight community volunteers.

An order that is written by your doctor. It instructs healthcare providers not to do cardiopulmonary resuscitation (CPR) if your breathing stops or if your heart stops beating.
What is a Transportable Physician Orders for Patient Preference?
A physician order that communicates your wishes regarding CPR, ventilation and other life sustaining treatments. This is transportable across all settings if you have a life-threatening illness.

Chapel and Meditation Garden
The chapel is located in the northwest part of the hospital on the first floor next to the pastoral care offices. It is a sacred space available for prayer and reflection. A lovely meditation garden is accessible directly outside the chapel. Sacraments are arranged upon request.
Pastoral Care
The Pastoral Care Service strives to be a channel of peace, offering compassion and support and the healing power of God to patients, their families and hospital associates. This is done mainly through direct patient visits, the sharing of sacraments and the offering of prayer and spiritual counsel. If the patient desires, Pastoral Care will contact his or her parish, congregation or clergy person. Finally, Pastoral Care works as an integral part of the healthcare team to provide meaningful, quality patient care.
Any patient or family member may call Pastoral Care by dialing 785-623-5000 and asking to speak with a chaplain.

Room Service
HaysMed is pleased to offer traditional room service for patient meals. Upon admission, you will receive a copy of our Patient Room Service Menu. When you would like to eat, simply dial 3663 (FOOD) on your room phone to place your order using this menu.
Patient and guest meals can be ordered any time between 6:45 a.m. and 6:30 p.m.. Meals will be delivered within 45 minutes. Patients who have been placed on a special diet may have restrictions on their selections.
Family members or designees may place meal orders for patients by calling 785-623-5999. For patients who are unable to participate in patient room service and do not have a family representative available to assist them, meal selections will be made by the Nutrition services staff. Please contact Nutrition Services extension 3663 from in the room and 785-623-5999 from an outside line.
Guest Trays
Guest trays consisting of soup or salad, one entree, a dessert and beverage are available for patient guests at a cost of $7.00. Credit Card, checks, and cash are accepted. The hostess will process the payment when the tray is delivered.

Kind words and thoughts can brighten a friend or loved one’s day when they are in the hospital. That’s why HaysMed gives you the opportunity to let someone know you are thinking of them and wishing them well.
To send a message, just fill out the form and we will print and deliver a copy. Messages are usually delivered within 24 hours but can take longer if sent over weekends or on holidays. Please note that mail deemed inappropriate by HaysMed staff won’t be delivered.
We do not have the ability to forward patient emails to patients if they have already been released from the hospital when the message is received.
Fitness Trails
Indoor Walking Trail: Take a break while at HaysMed and take advantage of one of our beautiful walking trails. We have an Inside Walking Trail located on the first floor of the hospital. Maps are available in the lobbies of entrances A, B and C. We also have a scenic Outdoor Walking Trail.
Outdoor Walking Trail: HaysMed’s Fitness Trail is located on the east side of the hospital campus. The paved portion of the trail is six-tenths of a mile long and there is an additional four-tenths of a mile long grass path nature trail. Alongside this tree-lined trail is a picnic shelter and playground equipment. Numerous benches allow for a break from your walk or jog, and a gazebo is located on the east side of the trail to allow for shaded cover from the sun.
Whether you are heading out to the trail for a morning jog, leisurely stroll or for a birthday party at the playground and picnic shelter, the HaysMed Fitness Trail is a great place to be! For more information, please feel free to contact us!

Medical Records
HaysMed is happy to provide a copy of your medical records for follow-up care, insurance purposes or your own use. There is no charge for release of medical records. For more information, you can reach us at 785-623-5824.

Patient Rights and Responsibilities
The basic rights of Patients, including neonates, adolescents and/or guardians, for independence of expression, decision, actions, and concern of personal dignity and human relationships shall be respected and observed by all hospital staff. Reasonable and responsible behavior on the parts of Patients, their Relatives, and Friends is expected at all times.
Patient Rights
HaysMed and the medical staff have adopted the following statement of patient rights. This list shall include but not be limited to the patient’s right to:
- Receive a written statement of his/her rights as a patient in advance of, or when discontinuing, the provision of care. The patient may appoint a representative to receive this information should he/she so desire;
- Exercise these rights without regard to sex or cultural, economic, educational or religious background or the source of payment for care;
- Considerate and respectful care, provided in a safe and secure environment, free from all forms of abuse or harassment;
- Remain free from seclusion or restraints of any form that are not medically necessary and are used as a means of coercion, discipline, convenience, or retaliation by staff;
- Be provided with the name of the physician who has primary responsibility for coordinating his/her care and the names and professional relationships of other physicians and healthcare providers who will see him/her;
- Receive information from his/her physician about his/her illness, his/her course of treatment and his/her prospects for recovery in terms that he/she can understand;
- Receive as much information about any proposed treatment or procedure as he/she may need in order to give informed consent or to refuse the course of treatment. Except in emergencies, this
- information shall include a description of the procedure of treatment, the medically significant risks involved in the treatment, alternate course of treatment or non-treatment and the risks involved in each and to know the professional status of the person who will carry out the procedure or treatment;
- Participate in the development and implementation of his/her plan of care personally or by his/her representative, and actively participate in decisions regarding his/her medical care. To the extent permitted by law, this includes the right to request and /or refuse treatment;
- Formulate advance directives regarding his/her healthcare, and have hospital staff and practitioners who provide care in the hospital comply with these directives (to the extent provided by state laws and regulations);
- Send and/or receive visitors, mail, telephone calls or other forms of communication with restriction. If restrictions are required for therapeutic reasons, the patient and/or family will be informed of the rationale for restrictions.
- Have his/her family representative, and/or personal physician notified promptly of his/her admission to the hospital;
- Be provided with full consideration of privacy concerning his/her medical care program. Case discussion, consultation, examination and treatment are confidential and should be conducted discreetly. The patient has the right to be advised as to the reason for the presence of any individual involved in his/her healthcare.
- Confidential treatment of all communications and records pertaining to his/her care and his/her stay in the hospital. His/her written permission will be obtained before his/her medical records can be made available to anyone not directly concerned with his/her care;
- Access information contained in his/her medical record within a reasonable time frame (usually within 48 hours of request);
- Be provided with reasonable responses to any reasonable request he/she may make for service within the hospital’s capacity, it’s stated vision, and applicable law and regulations;
- Leave the hospital even against the advice of his/her physician;
- Expect reasonable continuity of care and to know in advance the time and location of appointment as well as the physician providing the care;
- Be advised of the hospital grievance process, should he/she wish to communicate a concern regarding the quality of the care he/she receives or if he/she feels determined the discharge date is premature. Notification of the grievance process includes: Whom to contact to file a grievance, and that he/she will be provided with a written notice of the grievance determination that contains the name of the hospital contact person, the steps taken on his/her behalf to investigate, the results of the grievance and the grievance completion date;
- Be advised if a hospital/personal physician proposes to engage in or perform human experimentation affecting his/her care or treatment. The patient has the right to refuse to participate in such research projects;
- Be informed by his/her physician or a delegate of his/her physician of the continuing healthcare requirements following his/her discharge from the hospital;
- Be advised that with the patient’s permission, family members are encouraged to participate in care decisions;
- If the patient has the legal capacity to exercise his/her rights, the patient has the right to exclude any or all family members from participating in his/her care decisions;
- Examine and receive an explanation of his/her bill regardless of the source of payment
- Know which hospital rules and policies apply to his/her conduct while a patient;
- Have all patients’ rights apply to the person who may have legal responsibility to make decisions regarding medical care on behalf of the patient
- Be provided with information necessary to enable him/her to make treatment decisions that reflect his/her wishes and that they will be supported by hospital personnel;
- Have clinical decisions concerning his/her care to be based solely on identified healthcare needs, without regard to the cost of the services or treatment or the patient’s ability to pay;
- To optimize comfort and dignity through treatment as desired by the patient or surrogate decision maker and acknowledgement of the psychosocial and spiritual concerns of the patients and family regarding dying and the expression of grief by the patient and family;
- Participate in the consideration of ethical issues by the HaysMed Bioethics Committee that arise in the care of the patient;
- Be transferred to another facility when medically permissible;
- Arrange payment of the portion of his/her bill not paid by third party coverage;
- Has the right to access protective services;
- Except appropriate assessment and management of pain
- Be advised of pastoral care/spiritual services
Patient Responsibilities
The care a patient receives depends on the patient himself. Therefore, in addition to these rights, a patient has certain responsibilities. These responsibilities should be presented to the patient in the spirit of mutual trust and respect. The patient has the responsibility to:
- Provide accurate and complete information concerning his/her present complaints, past illnesses and hospitalization, and other matters relating to his/her health
- Report unexpected changes in his/her condition to the responsible practitioner;
- Be responsible for making it known whether he/she clearly comprehends the course of his/her medical treatment and what is expected of him/her;
- Be responsible for keeping appointments and for notifying the hospital or physician when he/she is unable to do so;
- Be responsible for following hospital policies and procedures;
- Be responsible for being considerate of the rights of other patients and hospital personnel;
- Assist in the control of noise and the number of visitors;
- The patient is responsible for assuring that the financial obligations of his/her hospital care are fulfilled as promptly as possible;
- Inform hospital administration of any complaint or concern he/she may have regarding the delivery of quality of healthcare;
- If the patient has Advance Directives, it is the patient’s responsibility or that of his/her representative to present the Advance Directive to HMC staff.

Your Patient Portal
Your patient portal gives you direct online access to portions of your electronic medical record where your health care provider stores your health information. Your diagnostic test results, appointment information, prescription medications, immunizations, and more are all securely stored in this electronic record for quick retrieval – so you see what your provider sees!
Your patient portal also provides new, convenient methods of communication with your provider’s office. Send messages and schedule/cancel appointments (where applicable) – all online.
There are so many ways to enroll in our patient portal, so you make the choice:

- The next time you have an appointment, ask for help signing up. You will be given an authorization/enrollment form.
- You can also call 785-623-5151 to enroll in the patient portal over the phone.
Once your account is created, you will receive an email message with instructions for setting up your patient portal log in ID, password, and security question.

Why is Open Access Important to Patients?
Open access to your medical record is a way for you to become more informed and involved in your care. HaysMed invites you to review your visit notes written by your doctor, nurse or other clinicians involved in your care through our convenient patient portal.
Having the chance to read and discuss clinic visit notes with your physician or family member can help you take better control of your health and health care. Patients with access to their doctors’ notes feel more in control of their care and gain a better understanding of their health. Chances are that you will have a better recall of your care plan and you are also more likely to take your medications as prescribed.
Portal Information for Minor Patients Age 0-11 Years Old
For access to a minor patient’s patient portal, a parent or legal guardian may be granted full access until the minor reaches age twelve (12) years. On the patient’s twelfth (12th) birthday, the parent or legal guardian’s access will automatically be terminated.
The parent or legal guardian may sign up for proxy access at the child’s next appointment or by calling 785-623-5151. A minor child age 12-17 years old may grant their parent or legal guardian proxy access by completing the form: Patient portal Proxy Access Request for Minor patients (complete page 1 and section A).
Proxy Access
A proxy is a person who can access another patient’s Patient portal account information as if they were the patient. A proxy can be any one of the following:
- Parent of a child between the ages of 12-17 years old
- Spouse
- Caregiver
- Child
- Legal Guardian
An adult patient can request proxy access for another individual by completing the form entitled Patient portal Proxy Access Request for Adult Patients. Legal guardians or caregivers to adults with diminished capacity can request proxy access using the same form. Appropriate legal documentation verifying the relationship of the proxy to the patient must be provided.
A minor child age 12-17 years old may grant their parent or legal guardian proxy access by completing the form entitled Patient portal Proxy Access Request for Minor Patients. The minor, aged 12-17 years, must complete page 1 and Part B to authorize proxy access.
Revoking Proxy Access
At any time, a patient may request to revoke proxy access previously granted. The Proxy Access Revocation Request can be completed and returned to the hospital or clinic. The request may also be completed over the phone by calling 785-623-5151.
Pharmacy
Medical Pavilion Pharmacy is a full-service pharmacy at HaysMed. The pharmacy is located at 2214 Canterbury Drive.

Closed Saturday & Sunday
Pharmacy services are available to HaysMed patients and employees, as well as anyone in the community. We accept most insurance plans and worker’s compensation prescriptions, as well as Medicare and Medicaid. The pharmacy can special order items that are not stocked and also offers mail-out prescription services.
To have your prescriptions transferred to the Medical Pavilion Pharmacy, please use this transfer form. You may fax or email the completed form to the pharmacy. You may also call the pharmacy if you prefer.

Through HaysMed’s Bariatric Surgery Program, Medical Pavilion Pharmacy offers Celebrate Bariatric Vitamins to patients who have undergone these procedures. Due to the stresses that occur during surgery and the various physiological changes that result from the procedure, supplementation with vitamins and minerals is necessary for proper healing and good health.
Regular adult or children’s multivitamins do not effectively meet the needs of bariatric patients. Celebrate products were designed specifically to meet the requirements of these patients and make supplementation easy. Daisy Award
The DAISY Award is a nationwide program that rewards and celebrates the extraordinary care delivered by nurses every day. DAISY Award honorees personify the mission of HaysMed by consistently demonstrating excellence through their clinical skills, expertise, and his/her extraordinary compassionate care.
HaysMed is proud to be a DAISY Award Hospital Partner, recognizing one of our nurses with this special honor quarterly.

The HealthCare Champion program recognizes the CARE in healthcare. Each month, patients and their families send heartwarming stories about special ways their lives were impacted by a member of the HaysMed HealthCare team. Caring Hands PCT of the Year Award
The Caring Hands PCT of the Year award was established in 2007 to recognize those caregivers that provide the most intimate care at the bedside. Criteria for the award include promotion of teamwork and function as a team player, contribution to customer service, and participation in community and/or hospital affairs.
Candidates are nominated by peers or managers and should be employed at HaysMed for a minimum of two years at time of presentation. The winner will receive recognition during Nurses Week and a monetary award.
Debra Boeken HUC of the Year Award
Established in 2008, this award honors Debra K. Boeken, Health Unit Coordinator for many years at HaysMed. Criteria for the award promote the values that Debra showed in her daily work. Those criteria include promotion of team work and function as a team player, excellent customer service, and participation in community and/or hospital affairs.
Lanita Smith LPN of the Year Award
The Lanita Smith LPN of the Year Award was established in 2008 to honor a Licensed Practical Nurse that possesses the qualities of Lanita. Lanita was an exemplary nurse that started as an LPN and continued her education to be a MSN at the time of her death. The winner is selected based on criteria which include job knowledge and duty performance, contribution to customer service, self-development accomplishments, and participation in community and/or hospital affairs.
Candidates for the award are nominated by their peers or managers. They should be employed at HaysMed for a minimum of two years at the time of presentation.
The winner receives recognition during Nurses Week and a monetary award.
Sr. Mary J. Mollison Nurse of the Year Award
The Sister Mary J. Mollison Nurse of the Year award was established in 2003 to recognize the years of dedication and leadership Sr. Mary J. Mollison, RN, gave to St. Anthony Hospital and HaysMed. She served as Vice President of St. Anthony Hospital and the founding Board of Directors of HaysMed. Sr. Mollison was instrumental in the merger of the two facilities.
This is awarded yearly in March at the HaysMed Associate Service Banquet. The candidate must be a Registered Nurse employed at HaysMed for a minimum of five years and employed at the time of presentation. The candidate may be nominated by any HaysMed Associate.
Recipients of this award receive an individually engraved award and $500.00 in cash. Their name will be featured on the Sr. Mary J. Mollison Nurse of the Year Award plaque displayed at the Med Center.
Visiting Hours
Visiting hours at HaysMed are from 8:00 a.m. to 8:30 p.m. daily. Many patients are in therapy sessions in the morning and afternoons, so please adjust your visiting hours to coincide with the patient’s schedule.
The main entrance doors to the hospital are locked at 8:30 p.m. Those include:
- Entrance A Bickle Pavilion/DeBakey Heart Institute
- Entrance B Miller Medical Pavilion
- Entrance C Main Hospital entrance
The Emergency Room Entrance is open 24/7.

HaysMed Volunteers are vital members of the HaysMed healthcare delivery team. Our volunteer program is designed to promote and advance the welfare of HaysMeds services and programs.
