Chronic Care Management
HaysMed offers ChartSpan, a care management program designed to offer exceptional support for eligible Medicare patients. Developed in partnership with the Centers of Medicare/Medicaid, ChartSpan provides round-the-clock access to a team of medical professionals, ensuring our eligible patients receive the necessary support to maintain their health and avoid unnecessary emergency room visits.

Diabetes Solutions
Diabetes can lead to many adverse health conditions and even death, so it’s important to manage the condition properly. Together, our internal medicine physicians and providers deliver comprehensive diabetes self-management education as part of our commitment to a healthier community, and a healthier you.
We believe that education is the key to empowering you to better manage the disease. Our Diabetes Solutions program is accredited by the American Diabetes Association (ADA), a testament to its evidence-based strengths and effectiveness. It takes the support of a diabetes team (primary provider, diabetes educator, dietitian, exercise physiologist, behavioral health advisor, care partner, etc.) to achieve optimal control of diabetes.
Program Goals
Our Diabetes Solutions program aims to:
- Help participants become active in their own care and diabetes management
- Provide avenues for patients to be successful in achieving self-management goals
- Ensure our program focuses on the national standards of care according to the ADA and the American Association of Diabetes Educators (AADE)
- Provide interactive learning and up-to-date information on diabetes self-management
- Provide education to caregivers so they feel comfortable being a part of the patient’s support team. Caregivers are encouraged to join patients at no additional cost.
Educational Classes
Diabetes Solutions is a comprehensive diabetes education program that consists of two educational sessions. Each session is five hours and will run from 8:45 am to 2:30 pm (lunch is included). It is not required that you do the consecutive classes in the same month. Click here to visit our events section.

- Healthy Eating
- Being Active (Exercise)
- Monitoring Your Blood Sugars
- Diabetes Medications
- Problem Solving (What to do when your glucose is high or low, sick days, etc.)
- Reducing Risks (Complications)
- Healthy Coping
Consultations
Our Diabetes Solutions program also includes these important learning opportunities:
- Private consultation with a certified diabetes educator for diabetes education, insulin starts, medication adjustments, pump starts, etc.
- One-on-one consultation with a dietitian for medical nutrition therapy
- Gestational diabetes consultation (for diabetes that occurs during pregnancy)
If you are interested in a Diabetes Solutions class and/or consult, ask your primary provider to fill out the referral form, sign it and fax it back to us. Our staff will then contact you for next steps.
About Gestational Diabetes
Are you pregnant and recently diagnosed with gestational diabetes? Or perhaps you were already diagnosed with diabetes and are trying to conceive or recently found out you are pregnant? You probably have a list of questions either way. We can help!
We feel very strongly that the more knowledge you obtain about diabetes, the more involved you will become in your diabetes management. This, in turn, will help you gain better control. We offer individual consultation with one of our experts, who will educate you about gestational diabetes and formulate a treatment plan specific to your needs. You will also meet one-on-one with a registered dietitian to receive information about dietary recommendations. The following topics will be discussed:
- What is gestational diabetes?
- Goals of medical nutrition therapy for gestational diabetes
- Dietary guidelines for gestational diabetes
- Pathophysiology of gestational diabetes
- Maternal/fetal effects
- Signs, symptoms, testing, and treatment
- Labor, delivery, and postpartum
- Glucose monitoring
- Insulin administration
- Exercise
Getting in for a visit is easy: Ask your OB/GYN or primary provider to fill out the referral form, sign it, and fax it back to us. Our staff will contact you. Education is offered by individual consult only with a referral.

Medical Records Release
We are happy to provide a copy of your medical record for follow-up care, insurance purposes or your own use. For Continuing Care, there is no charge if the records are sent directly to another care provider. Please enter the care provider’s name and address on the authorization form in Section 3.
If your medical records are for personal use, a cost is assessed at 25 cents per page.
